The form is brief, and rather simple, but you must fill out ALL parts. As for having a last name, IT IS MANDATORY. You must join all packs in character. This means you start out as a loner, but if there is land available, you may claim it c: For birthday, put month then years. e.g. march, year 2. At least four bad and good traits.
